This screen displays a form field to enter the email address used during signup. After entering the address, click the "Send" button to submit the form.

If the email address as entered is not found or contains a validation problem, this screen will reload and indicate the error which needs to be resolved in red text to the right of the field. Once the submission is free of errors, a password reminder will be sent to the address.
